2024 APRIL LETTER– current information to prepare for the summer of 2024
Detailed Instructions to help you complete your campers’ forms

Online Forms Required for Everyone
Camper Health Questions: choose your campers meal plan, let us know about their last physical and if they will be taking any medications while at camp.
New Camper Cabin Mate Request: New campers may request one cabin mate of the same grade. The request needs to be mutual. Returning campers may use this form if they are requesting a new camper only.
Medical Form: Questions about your campers health including: allergies, medications, immunizations, conditions, medical insurance and your signature giving us permission to treat.
Optional Online Forms
Monthers Visiting Day Plan: If your camper has been accepted for a month, please let us know if you are planning to take them out of camp that day or if we should include them in our plans for the day.
Camper To Be Picked Up by Someone Other Than Parent Permission Form: If your camper will be picked up by someone other than the parent, camp needs to have this form completed to release the camper to that person. That person is required to be 21 or over.
Birthday Cake/Huckster Ceremony Sign Up: Will your camper be having a birthday or will they be a Huckster this summer? Use this form to sign up for a cake or to celebrate their 5th year at Camp Huckins.
Required Documents to Upload
Camper Photo: If you did not upload a photo during the application process you can upload one here. This photo will be on the contact list given to families at the end of the session to help everyone stay in touch.
Physician’s Examination Form: All campers need to have a physical within one year of the date they attend camp. You may download our generic form and then upload the completed form to your dashboard. You may also upload your own doctor’s form instead.
Immunizations: Be sure to upload your child’s immunization dates. You may download the generic form or upload a copy of your own doctor’s form. Please remember it can only be one file. Feel free to use smallpdf.com or call us for help to get multiple pages uploaded.
Medical Insurance Card: If your camper is covered by medical insurance please upload a copy of the front and the back of the card.
Documents required for A Camper that takes Medication
Inhaler or Epi Pen Form: If your child may need an epi pen or inhaler during camp, the state of NH requires this form to be filled out and signed by you and your doctor. This form is due by May 1st.

Tips for Uploading Multiple Pages
Only one file can be uploaded for each form. If you do not have a scanner and need help converting jpegs to pdfs you can use smallpdf.com. This site will allow you to create two pdfs an hour at no charge. For example: You have a paper copy of your child’s physical which is three pages. You can take a photo of each page with your cell phone (or camera), email them to yourself and save them on your computer. Then open smallpdf.com and choose Jpeg to Pdf. You can choose your three photos and it will merge them into one pdf that you can then upload to your dashboard.
