Detailed Instructions to help you complete your camper’s forms
Important Information About Medical Forms
All health histories and medical forms are due by May 1st with NO exceptions. With 1300 campers and over 150 staff members, we need time to individually review each form before camp starts so that we can focus on camper health during your child’s time at camp. If your forms are not done by May 1st, your child may lose their spot at camp. Please reach out to the office if you will have any difficulty meeting this deadline. This policy is to ensure the safety and well being of all campers. Thank you for your cooperation.
Please note:
● We require a yearly physical exam to attend camp
● We require the provider’s electronic signature or hand written signature on the physical exam form
● We do not accept health summaries that do not have a signature
● If your child has a physical exam after May 1st, please upload their most recent exam form now and update Camp Brain with the new physical form after the exam is done. Please also note in the health history when their upcoming exam will be so we can note it in our records.
● All medications your child is taking at camp MUST be listed on their medical form and physical form.
If it is not included on the form, we require a signed note or medical order from your provider indicating the medication is to be taken at camp. Medications that must be listed include:
○ Prescription medications
○ As needed medications such as inhalers, epi-pens, and migraine rescue medications
○ Over-the-Counter medications
○ Vitamins
○ Supplements
MEDICATION
If your child is taking medication at camp, you must list all medications in Camp Brain under
health history and order all medications through PackMyRx.
Medication Ordering Instructions
Please Note-We do NOT allow gummy medication. These cannot be properly and hygienically
stored and dispensed at camp. Please practice with chewable tablets or pills before
coming to camp. There will be NO exceptions.
On-Line Medical Form- Required for all Campers
Please complete all sections of the Online Medical Form including: Allergies, Medications, Immunizations, Health History, Medical Insurance and Permission to Treat Waiver.
Copy of Last Physical- Required for all Campers
Please upload a copy your camper’s most recent physical even if it will be more than a year old.
Your medical professional’s form is fine. If they need a template please use the template below.
Copy of Immunizations- Required for all Campers
Please upload a copy your camper’s Immunization Dates.
Your medical professional’s form is fine. If they need a template please use the one below.
Physicals dated after May 1st 2026
If your camper will have a physical between May 1st and their arrival at camp please upload a copy.
Inhaler or Epi Pen Form
If your camper requires an epi-pen or inhaler this form needs to be completed and signed by both a Parent/Guardian and medical professional.
